2. Do not fill out this form until you are ready to submit, because it cannot be saved.

1. Club president or treasurer submits this form.
2. Club president or treasurer hands all money and receipts to a Student Development staff member at the Student Development Office in SSC 207, Monday through Thursday, 9am to 5pm.
3. The Student Development staff member will count and confirm your deposit.
4. Keep track of your deposit information on your Budget sheet in Dropbox.
