LEVEL 2 CLUB APPLICATION
All requirements (including Kognito Certificates) must be completed during the 2017-2018 school year.
Steps for Requesting Level 2 Club Status:
1. Your club's president, vice president and treasurer have attended the Level 2 Club Training.
2. At least 5 members (including the president, vice president and treasurer) have completed Kognito Training and you have submitted the completion certificates (completed in current semester) to the Student Development Office via email.
3. Complete this form and wait to hear back from the Student Development Office!
